New Patients:
After we have scheduled your first appointment you will need to fill out a new patient intake form by 1 of 3 ways. (1) Online Intake Form - If you provided an email upon scheduling, you will receive an email 36 hours prior to your appointment from Practice Fusion with a link to complete the patient intake form. You will need to create an account with Patient Fusion in order to complete the online intake form. (2) Paper Intake Form - Download the new patient forms from the "Patient Resources" section below and hand deliver them at least 15 minutes prior to your appointment. (3) Arrive at least 30 minutes prior to your appointment and we will provide you with paper intake forms to complete.

Our clinic is conveniently located at 1607 S. Muskogee Ave. Ste. C in Tahlequah, OK. The main entrance is shared with Cherokee Hills Pharmacy on the south end of the building. For directions, use the Google map on this page or call our office at (918) 772-5456 to speak with a member of our staff.

If you are experiencing a medical emergency, please call 911 or present to your nearest emergency room for evaluation and treatment.

Appointment Cancellation:
Please be sure to cancel your appointment at least 24 hours in advance to avoid being charged a cancellation fee. This helps us maintain open slots for same day appointments to better serve you. We charge $25 for missed appointments.
